A restaurant I worked in once imported some fresh porcini mushrooms directly from Italy. We ran a porcini risotto as our special for the week. They used domestic porcini's for the bulk of the risotto but would garnish the plate with the imported mushrooms. It was one of those restaurants where you deliver the food on a plate with a lid and then remove the lid once the food is in front of the guest.

I brought out a dish of the risotto to the table, they were sharing it as an appetizer so it went square in the center of the table. I unsheathed the dish and atop the risotto were the mushrooms....atop of the mushrooms were about 10 little worms squirming about. They were definitely in the maggot family. I quickly put the lid back down and mumbled... "ummm, they forgot the garnish" and walked away. Nobody else noticed, it was amazing.

We tossed the mushrooms and switched the special. Several of the staff had eaten some earlier and we'd served it to many guests. It was pretty gross.
